Spravato Treatment for Depression in NYC

Medically supervised Spravato® (esketamine nasal spray) treatment for adults with treatment-resistant depression, available in Manhattan.

Depression that doesn’t improve after trying multiple medications can be frustrating and difficult to live with. For some patients, Spravato offers a different option than standard antidepressants.

At Wele Clinic in Manhattan, Dr. Mohammadmehdi Omidvari assists adults considering Spravato treatment for treatment-resistant depression. During a consultation, we review your treatment history, talk about how Spravato works, and explain what happens during each visit. The goal is to give you clear information and help you decide if this approach feels right for you.

What Spravato is

Spravato is a prescription nasal spray containing esketamine. While many people search for ketamine therapy for depression, Spravato is the specific FDA-approved ketamine drug for depression designed for clinical administration. It’s not a replacement for your regular oral antidepressant, but should be taken along with it.

Spravato doesn’t work like traditional antidepressants that affect norepinephrine, serotonin, and dopamine. Instead, it affects the glutamate receptors in the brain. This is why it’s often prescribed by doctors when other medications haven’t worked.

Who this is for

You’ve been through the cycle: Zoloft, Lexapro, Wellbutrin... and the relief still hasn't come.

This is known as treatment-resistant depression—when you’ve tried at least two different antidepressants without meaningful improvement.

This nasal spray depression treatment is intended for adults in this situation. It works differently than traditional antidepressants and is administered in the clinic under medical supervision.

Why Spravato for Treatment-Resistant Depression

Switching medications has its limits

Eventually you will reach the point of just changing names, not mechanisms, as most antidepressants are within the same class, leading to diminishing returns.

This is why Spravato exists

After you have tried multiple antidepressants and nothing is working, it’s clinically reasonable to try a different approach.

Research-backed improvements

It doesn’t work for everyone, but clinical research has shown a significant symptom improvement for some patients.

Ongoing monitoring & adjustments

This is a standard part of care. Treatment decisions may be adjusted over time depending on how you respond to the treatment.

Why Wele Clinic

I take the quality of care just as seriously as the treatment itself, from the initial consultation through ongoing sessions.

I carefully evaluate each patient’s eligibility before treatment begins and remain hands-on throughout the process. Every step is thoughtfully structured, with medical supervision and monitoring during and after treatment.

Clear expectations around safety, the sessions themselves, and long-term care are established from the start. All treatment is overseen by me as a board-certified psychiatrist.

Insurance & Eligibility

Insurance coverage is a very common question and eligibility and coverage will vary by plan.

  1. Typically Spravato treatment is considered for patients with commercial insurance, and depending on your plan, out-of-network benefits may apply, but this depends on your individual policy details.

  2. Costs and coverage are discussed in advance and our team will help you understand your options so you aren’t surprised by anything after the fact.

  3. All the details you need to know are reviewed early in the process—a consultation is the best way to review eligibility.

What Our Patients Say

Frequently Asked Questions

  • If you have tried at least two different antidepressants at adequate doses and durations, but there hasn’t been any meaningful improvement, then you would likely be eligible. However, this is determined by your current symptoms, overall health, and treatment history, which is discussed during a consultation.

  • No. Spravato is intended for those suffering from treatment-resistant depression. It is not a first-line treatment, and is considered when you haven’t received relief from standard antidepressants.

  • Insurance coverage for Spravato varies by plan. During the consultation, our team will help you to understand what may be covered before treatment begins. However, approval typically depends on medical necessity criteria, individual policy details, and your prior treatment history.

  • Coverage under Medicare or Medicaid plans can vary and may be more limited. This depends on your specific plan.

  • We review cost-related details before your consultation and won’t begin a treatment without first clearly communicating with you around financial expectations. The costs themselves depend on factors like deductibles, whether out-of-network benefits apply, insurance coverage, etc.

  • Spravato is FDA-approved and must be administered in a certified medical setting. At Wele Clinic, treatment is medically supervised and includes structured monitoring during and after each session to ensure patient safety.

  • Side effects may include fatigue, increased blood pressure, nausea, dissociation, and dizziness. These may present themselves during or shortly after treatment. This is why it is required for you to stay at the clinic after treatment so we can monitor the side effects closely during each visit. Any potential risks and side effects will be discussed with you during your consultation.

  • While mainly used for treatment resistant depression, esketamine for bipolar depression is a treatment pathway we can discuss during your consultation.

  • Some patients notice symptom improvement earlier than with traditional antidepressants, but responses vary. Spravato is not a guaranteed solution, and treatment decisions are reviewed regularly based on progress and tolerability.
