A note on controlled-substance registration
Ketamine and esketamine are controlled substances, and administering them requires a federal registration. We deliberately do not publish that registration number anywhere on this site, in any document, form or image. It is not held in a public register, and a published registration number can be used to write fraudulent prescriptions. Regulators, certification bodies, pharmacies and payers that need it receive it directly from us on request.
States where we can treat you
A physician may only treat you in a state where that physician is licensed, and what counts is where youare sitting at the time of the visit — not where you live, and not where we are.
- In person: New York only, at our office at 1123 Broadway, Suite 1117. All Spravato and all IV ketamine treatment is in-person care and is available only here.
- Telehealth: only to patients who are physically located in New York at the time of the appointment, which is where Dr. Omidvari is licensed.
If you are outside those states when your appointment is due, we have to reschedule it rather than see you. Tell us as early as you can and we will find another time. Reaching us first

Raising a concern with us never affects your care, and it never affects your ability to take the same concern to a regulator. You can do both, in either order.
Reporting a side effect or an adverse event
If something happens after a treatment — a side effect, an unexpected reaction, anything that worries you — call the clinic first on (646) 874-9340. We need to know so we can look after you and so we can adjust your treatment plan.

Filing a privacy complaint
If you believe your health information has been used or disclosed improperly, our Notice of Privacy Practices sets out how to complain to the practice and how to complain directly to the U.S. Department of Health and Human Services, Office for Civil Rights, including the address, phone number and online portal. We will not retaliate against you for filing a complaint, with us or with them.
Raising a concern about a physician
